There are countless individuals in the community who are not necessarily involved in the Clubhouse Day Program, but still need support to reduce their chances of relapse and hospitalization and the Crisis Prevention Program (CPP) is available to meet their needs.  CPP offers community support services to these individuals and utilizes a comprehensive team approach to ensure that adequate support and services are available to them so they can achieve their goals and aspirations. Support is offered 24 hours a day so that individuals have the opportunity to live an enriching life outside hospital walls. In response to the increased demand for this type of service, Liberty Centre Services has expanded their community support program to include Intensive Community Support (ICS) and Recovery Support Services. The addition of these services will enable individuals to receive the level and type of support they need in a timely manner. Community Support services can include transportation, medication management, residential support, leisure activities, illness education, symptom management, assistance with financial needs (e.g., budgeting), and assistance with basic needs such as food, clothing, and shelter.

Liberty Centre Services recognizes the significant correlation between good physical health and good mental health, which is why the health services provided by nurses in the Crisis Prevention Program have been well utilized this year. Nursing services include education, medication management, physical assessment and treatment, and guardian or family contact; a nurse will also accompany members to doctor appointments if needed.
